Advocating for People with Disabilities

Goodwill’s trained Targeted Case Management team realizes that finding resources and navigating through all the options can be overwhelming. Through the Targeted Case Management program, Goodwill provides support and advocacy services for people with disabilities by developing, coordinating and monitoring services, supports and resources.

Individual case managers will assist with funding resources, applications, and community supports such as:

  • Assessments
  • Finding and securing financial assistance for necessary services and/or technologies
  • Person Centered Support Service Planning (PCSP) and coordination
  • Information and referral
  • Facilitating community inclusion
  • Discharge planning
